Supplemental Payer Errors for Medicare

CMS released on March 1st that they have identified a problem that requires IMMEDIATE action for Medicare Part B providers. Apparently, claims were not crossing over to supplementary/secondary payers from January 5th to February 12th. Certain criteria applies to claims not forwarded.

Ophthalmology Pricing Update

CMS has clarified pricing for Ozurdex (dexamthasone intravitreal) when billing HCPCS Level II code J3490. Ozurdex is an implant injected into the vitreous to treat adults with macular edema. Payment has been incorrect and should be reviewed.

Preliminary 2011 Payment Policies

On February 19th the "Advanced Notice" was released by CMS (Centers for Medicare and Medicaid Services) notifying health plans of the calculation of capitation rates for Medicare Advantage organizations for 2011 and changed the methodology. The notice also addressed policy and payment methodologies for Medicare Part C and D.

Medicare Pay Cuts

As of February 25th the Senate has passed the Temporary Extension Act of 2010 which extends the Medicare physician 21.2 percent pay cuts exemption and the physical therapy services cap exceptions until March 31st.

Revised CPT Codes

Codes 90664, 90666, 90667, and 90668 were accepted at the October 2009 CPT for the Category I vaccine product codes and will be effective July 1st. The CPT code 90670 Pneumococal conjugate vaccine 13 valent, for intramuscular use has been removed pending FDA Approval. Other effective and removed codes are available as well

What are Doctors Doing?

One 9 doctor Cardiology group had decided to confront Medicare cuts by charging annual patient fees ranging from $500 to $7,500 in addition to regular fees paid by patients and insurers. Physicians are not usually allowed to charge Medicare patients any fees in addition to the set Medicare rate for covered services like new patient evaluations, but they can charge separate fees for things that Medicare does not cover. They sent letters to their patients announcing the new annual fees. Patients could choose the level of extra services they want by purchasing one of 3 basic plans. For instance $500 a year plan would buy priority appointments and other basic services; $1,800 a year a patient would be able to email the doctor and schedule same day appointments; and for $7,500 a year a patient could get the doctor's personal cell phone numbers and call the doctor on nights and weekends. Some patients are not able to pay the for new fees, so this can be a very volatile and dire situation for many patients unable to pay the extra costs.

WEBeDoctor Updated Features

New Reporting
Through any role in WEBeDoctor you can now pull a new type of reports.
Go to "Patients" > "Patient List"
You can search through your existing patients by gender, DOB, ICD or Diagnosis Code, Medication, and even State and Zip Code.
It shows all your patients that fit your search criteria.
All in a neat printable report.

Scheduling Update
Defaults have been changed in the Calendar in the Physician role or the Appointments through the Front desk
The default settings for the Calendar and Appointments have been changed to show
Cancelled, No Show, and Blocked Appointments by Default
They can be hidden by selecting the "Hide Denied Request" button on the top left corner.

Updated Superbills in EMR
Superbills in the EMR have been updated
Superbills now include diagnostic pointers.
Diagnostic pointers explain on the superbill and claim which procedure is connected with which diagnosis
This ensures correct codes used at the point of care for billing

Electronic Prescription Update
Electronic prescriptions have been updated
Newly added features show drug to drug interactions and drug to allergy reactions
This will give an automatic interaction description, which drugs and interactions, and the severity of the interaction or reaction.
This can also be ran separately by pushing "Run Validations" button as well

Allergy Update
The Allergy Section have been updated
We have added Allergy Classes for patient allergies
You can select particular medications through the drug database as well
You can enter the particular class of allergy OR medication, but they cannot be entered at the same time
Any medication prescribed with the particular class or medication will give a warning indication of allergy reaction or interaction.

Billing Update
The Superbill in the Billing Section has been Updated
When creating a Superbill by search the appointment changes have been made
You can search for your appointment by the doctor and date
Once you have selected and completed your Superbill you can open up the Search Appointment and your search will still be there
This will ensure you have billed for a particular doctor and date of service
